Please read this information carefully before signing.
At our practice, we collect your personal and health information so we can provide you with safe, high-quality medical care. We are committed to protecting your privacy and handling your information in accordance with applicable Australian privacy laws.
How We Use Your Information
Your personal and health information may be used or disclosed for purposes including:
- Providing your medical care and communicating with other healthcare providers involved in your treatment, including your referring doctor, specialists, hospitals, pathology and imaging providers, pharmacists and allied health professionals.
- Preparing referrals, prescriptions, medical certificates, reports and correspondence relating to your care.
- Administrative purposes necessary for operating the practice, including appointment scheduling, medical record management, billing, Medicare and private health insurance claims, accreditation, quality assurance and legal or regulatory requirements.
- Contacting you about appointments, test results, recalls, follow-up care or other matters relating to your treatment.
Clinical Photographs
As part of your medical care, clinical photographs may be taken where clinically appropriate, particularly for plastic and reconstructive surgery. These images form part of your confidential medical record and are used to assist with your assessment, treatment planning and ongoing care.
With your privacy protected, de-identified information or images may also be used for clinical audit, quality improvement, education or research, in accordance with applicable laws and ethical standards. Any use of identifiable images outside your clinical care will only occur with your separate consent.
AI-assisted clinical documentation
To improve the accuracy and efficiency of clinical documentation, Dr Fairbank may use secure AI-assisted dictation technology during consultations. This technology assists with preparing consultation notes but does not make clinical decisions or replace the doctor's professional judgement.
All clinical notes are reviewed and approved by Dr Fairbank before being added to your medical record. Information processed by the AI provider is managed using appropriate security measures and in accordance with Australian privacy requirements.
Your privacy
Our practice complies with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and the Health Records Act 2001 (Victoria).
You may request access to your health information or ask us about our Privacy Policy at any time. A copy of our full Privacy Policy is available upon request.
